About the Study

This study is testing whether precision screening—using a person’s genetics to guide screening—can make cancer testing more accurate and reduce harms. Participants will have genetic testing that looks for rare genetic variants and a polygenic risk score (PRS), which estimates a person’s inherited risk of cancer.

The study will use this genetic information to give Veterans and their primary care providers personalized screening recommendations. The goal is to find men at higher risk who may benefit from early testing, while helping men at lower risk avoid unnecessary procedures.

This Study is Open To:

Men 55-69 years old who:

  • Receive regular VA care
  • Are Veteran status

This Study is NOT Open To:

Men with the following are not eligible to participate:

  • Have a personal history of cancer
  • Have had a prior biopsy, prostatectomy, or MRI
  • Are confirmed to already have a rare genetic variant associated with cancer

What the Study Involves

Participants will be randomly assigned to one of two groups.

  • Precision Screening Group: Participants will receive a genetic risk report for cancer, personalized screening recommendations, and—if they are found to be at high genetic risk—genetic counseling. Their primary care provider will also receive the report and educational materials. High-risk men will be advised to get screening, while low-risk men will be advised not to screen.

  • Usual Care Group: Participants will receive a short brochure about shared decision-making in cancer screening.

All participants will be followed for a period of 7 years. During that time, researchers will track cancer diagnoses, tests, MRIs, genetic findings, screening preferences, and health-related quality of life.
